Transition Pathway

Before we accept a patient at Cawston Park, consideration of how we would be able to effectively transition that individual back to a less restrictive environment begins.

Our Behavioural Nurse Specialist, working as part of our multi-disciplinary team, will initiate a comprehensive assessment and information-gathering process and the transitional pathway will commence.

From the point of admission, the transitional pathway forms part of the individual’s overall care and treatment plan.

Our aim, being to return the person back to a more community-based service and setting, ideally in an environment of their choosing and with the skills and well-being to lead a sustaining and fulfilling life.
